Unless otherwise arranged, consultations sessions are scheduled for 50 minutes. Together we will agree on our frequency of meeting. Once per week, or every other week, are common frequencies. A scheduled appointment means that I reserve time only for you. If you miss your appointment or cancel with less than 24 hours notice, you will be billed the full fee. Generally, your insurance company does not pay this fee.

Unless otherwise arranged, my fee for a 50 minute appoinnnent is $115. The fee for 12 sessions of Group is $480. You are financially responsible for the full fee of $480, regardless of whether or not you attend a sessions. Services for time periods other than a usual appointment are charged
proportionally at $115 an hour. Examples of this include report writing, phone conversations beyond a few minutes, or consulting with others at your request. It is your responsibility to verify insurance coverage, copayment, deductible and policy limits. Your insurance company will be billed for your sessions. I will be paid directly by the carrier and you will be responsible for any deductibles and co-payments. If your insurance company determines you are not eligible, you are responsible for full payment. Payment arrangements should be finalized during your first visit. In the event of default of payment, the balance is due in full. You will be responsible for any reasonable court costs, attorney fees, and/or collection fees incurred.

Registration for group requires a deposit of $150.00. Refunds for group will be available upon a written request up to 14 days prior to the start of the group session minus a $40.00 processing fee. Fees will not be refunded if cancellation is made within the 14 days of the start of group. No refund will be given once the group session has begun.

It can be difficult to determine health plan coverage. Some insurance plans require authorization before they pay and may limit the number of visits. You have the right to request reconsideration if visits are denied certification. You have the responsibility to check with your insurance
company and to submit a grievance to your insurance company. You also have the right to complain to me about any aspect of treatment. If you feel that I am not meeting your needs, please talk to me and we can find a therapist who may better serve you.
